Yes! I was going to point out the same thing. I'll go back and check the tape, but it did look like a scar that had been stitched. (I suppose it could
be a Dharma shark bite, but it looked like a stitched wound) and the Doc definately did not have that or any facial wound when his throat was slit.
So who, what or when was the Doc that washed up on the Island? Doc from an alternate timeline?
