Well, if I remember correctly, and it's quite possible I don't, the wound even had various stages of healing in the episode where we first met him. I'll see if I can find the discussion later when I have more time.
I rewatched the episode at lunch and the doc did have the wound when Keamy slit his throat, he even turned his head to the side so we could see it..surely deliberate on the part of the director. It was mostly healed though and there were of course no stitches. It's hard to imagine how only one part of the doc's face could go through some sort of time warp lol
