A designer or advertising director can hire models of any size they like. Yes, there are zeros and twos, but there are also fours and sixes and eights and tens and twelves and fourteens... The idea that there is a limbo zone between regular size and plus size is more a Tyra creation than anything else. She needs a way to differentiate. Really, anything above a two is probably plus-size, but there is work* for mid-size girls (6-10) and big girls (16+). It's a range.
*I'm not necessarily talking high-end fashion work, but "real-person" print work and catalog work...
